A woman in her 40s with a strong family history of breast cancer and high-risk genetic mutation desired bilateral prophylactic mastectomy. Because of her breast size and asymmetry, she first underwent a reduction/mastopexy to allow for nipple sparing mastectomies. This procedure was followed several months later by bilateral nipple sparing mastectomy with immediate microvascular abdominal flap breast reconstruction. She underwent fat grafting to both reconstructed breasts secondarily. The post operative photographs were taken approximately 4 years after her last procedure.
