A woman in her 50s with breast cancer underwent bilateral mastectomies with tissue expander placement. Unfortunately, she developed significant skin flap necrosis requiring removal of the expanders. This was followed by radiation therapy to the left chest wall. She ultimately underwent successful bilateral microvascular abdominal flap breast reconstruction. She subsequently had minor revisions with fat grafting, skin resection and nipple reconstruction. The preoperative photos were taken after loss of the tissue expanders and radiation therapy. The postoperative photos were taken approximately 10 months after her most recent surgery.
