A woman in her 60s, seven months post-op complex reconstructive rhinoplasty. This had a massive skin cancer resection and previously underwent a skin graft of the nasal dorsum which was placed directly on the bone and cartilage. This resulted in significant dorsal asymmetry and indentation. She also had a dorsal hump, over-projected tip, tip asymmetry, and a drooping tip. Tip elevation, dorsal hump reduction, tip refinement, placement of a dorsal onlay graft consisting of mastoid fascia under the skin graft, and nasal tip deprojection were performed. The overall goal is a conservative change of her nose to result in a natural appearance of the nose which does not droop when smiling and improves breathing.
