What is a deductible?
A deductible is a part of the agreement that you have with your insurance company. Deductibles are a predetermined amount of money that you must pay before your insurance benefits will begin. Please refer to the terms of your insurance plan regarding deductible levels.
What is a co-payment?
A co-payment is a fixed fee that is required to be paid each time you receive care. A co-payment is the sole responsibility of the patient, your insurance will not pay for this fee. Please refer to the terms of your insurance plan for information on the specific co-payments that relate to your policy.
For example, if your insurance company requires a $15 co-payment, each time you see your primary care doctor, you will be expected to pay $15.
What is co-insurance?
Co-insurance levels are a part of the agreement that you have with your insurance company. When you get billed for a service, the insurance company will determine what cost (allowable charge) they feel is appropriate for the services that you received. The insurance company will then pay a percentage of that “allowable charge”.
For example, if your insurance company requires 30% co-insurance, for every $100 in allowable charges, you will be expected to pay $30.
What is an Explanation of Benefits (EOB)?
An Explanation of Benefits is issued by your insurance plan and explains your financial responsibility based on the terms of your insurance policy.
What is NO-FAULT?
If you were hurt in an automobile accident, your medical services/treatments may be covered under No-Fault.
What is Workers’ Compensation?
If you were hurt or had an accident at work, your medical services/treatment may be covered under Workman’s Compensation.
My insurance plan issued a check in my name, what should I do?
You should sign the back of the check and write your account number under your signature. Then write “make payable to” and include Long Island Plastic Surgical Group. Mail the check with a copy of your bill to the address listed on the bill. You should also include a copy of the Explanation of Benefits (EOB) so we can credit your account for the right amount.
