A woman in her 20s, who had two previous rhinoplasties which left her with an extremely drooping tip, an over-projecting elongated nasal tip, a bulbous tip, wide alar base, and a dorsal hump. She is now 2.5 years post-op from revision rhinoplasty with tip elevation, nasal tip deprojection to shorten the overall length of her nose, tip refinement, internal nasal sill excision to decrease the alar base width, and placement of a columellar strut graft and spreader grafts using MTF cartilage to add tip support, tip refinement, and improve her breathing. A mastoid fascia tip graft was also used for tip unification.
