A woman in her 20s, who had two previous rhinoplasties which left her with an overly-pinched appearance of her nasal tip, an over-projecting elongated nasal tip, alar retraction, and inability to breathe through her nose. She is now two months post-op from tertiary revision rhinoplasty with placement of alar contour grafts to correct alar retraction, nasal tip deprojection to shorten the overall length of her nose, and placement of a columellar strut graft and spreader grafts using MTF cartilage to add tip support and improve her breathing.
